Is the service user aware of the referral to the Community Mental Health Support Service?

Unfortunately, we cannot accept referrals for clients who have not given consent for the referral to be made on their behalf.

Please seek consent before making a referral.

Why are you referring the service user to the Community Mental Health Support Service?

Is the person receiving support from any other services? (E.g. Social Care, CTT, Talking Therapies)

Please note that we are only able to accept referrals from Talking Therapies and Community Treatment Teams as part of someone's discharge planning. We can also accept referrals for people who are on the waiting list for Talking Therapies

Does this person have any access needs? E.g. visual impairment, interpreter required etc.
