Who we're here for
We’re here for adults across Newcastle, supporting them with non-clinical needs that are affecting their mental and physical health, or social needs. This could be financial struggles, housing issues and isolation.
Our link workers work with people to improve their mental health and wellbeing, setting meaningful goals, building confidence, and accessing support in the community.
The support we offer
We support people on a 1-1 basis with:
- Finding benefit support/financial support
- Finding housing support
- Healthy eating and cooking
- Finding ways to get active
- Joining groups and taking part in activities
- Feelings of loneliness
- Employment support
- Long-term health conditions
How to access our support
If you’d like to be referred to our social prescribing service, please speak to your GP practice for a referral.
Our social prescribing teams are attached to four Primary Care Networks in Newcastle. These are:
- North Gosforth PCN – Park Medical Group, Brunton Park Health Centre, Broadway Medical Centre, Regent Medical Centre, Gosforth Memorial Medical Centre.
- Outer West PCN – Throckley Primary care centre, Parkway Medical group, Denton Park Medical Group, Newburn Surgery, Denton Turret Medical Centre, Westerhope Medical Group.
- Central PCN – Saville Medical Group.
- West End Family Health PCN – Cruddas Park Surgery.
